Oscar’s Pet Resort of Greenfield, Inc. Service Agreement This Agreement is entered into by and between Oscar’s Pet Resort of Greenfield, Inc. (Oscar’s Pet Resort) andOwner's Name* 1. I am the owner or agent of the dog/cat named on the application form, and I am authorized to sign this release form.* Yes No 2. I (Owner) have been provided and agree to Oscar’s Pet Resort’s current General Policies and hold responsibility for keeping myself updated of future policy changes. General Policies are available upon request and posted on www.oscarspetresort.com.* I agree 3. I (Owner) understand and agree that my pet(s) will, at all times, while attending Oscar’s Pet Resort, be in good health and maintain current vaccination status. I further understand that even if my pet(s) are vaccinated there is still a possibility of a dog or cat contracting an upper respiratory disease or other communicable disease/parasite.* I agree 4. My Veterinarian is…* … and I (Owner) give my vet permission to provide Oscar’s Pet Resort with necessary vaccination and health information pertinent to or which may be requested by Oscar’s employees providing care for my pets(s).* I agree 5. I (Owner) further understand and agree that in admitting my pet(s) to Oscar’s Pet Resort, Oscar’s Pet Resort has relied on my representations of my pet’s health and temperament and agree to notify Oscar’s Pet Resort immediately of any changes in my pet’s health and temperament. I understand Oscar’s Pet Resort is not responsible for veterinary bills for contagious illness that my dog might bring into or may pick up during my pet(s) stay.* I agree 6. I (Owner) agree to inform Oscar’s Pet Resort of any bite injury my pet has inflicted upon any human or animal. I (Owner) understand Oscar’s Pet Resort has authority to decline service to my pet in the event my pet has caused a bite to any human or animal.* I agree 7. I (Owner) understand and agree that all pets over six months of age attending Oscar’s Pet Resort group play are required to be spayed/neutered.* I agree 8. Oscar's Pet Resort will exercise reasonable care and precautions (see General Policies) and I (Owner) understand and agree that I am responsible for any harm caused by my pet(s) to any human, other pet, or property while my pet(s) are at Oscar’s Pet Resort and hereby assume any expense or liability for injuries my pet(s) may inflict on a human or other pet while receiving service at Oscar’s Pet Resort. Further, I (Owner) shall indemnify and hold harmless Oscar’s Pet Resort and its employees and agents from any and all claims, actions, cause of actions, lawsuits or damages, including any claims for attorney’s fees, which arise out of or are related to any action or actions of my pet(s) while my pet(s) are receiving any services at Oscar’s Pet Resort.* I agree 9. I (Owner) understand that dogs, when in off-leash playgroup, can accidentally injure each other. Minor cuts, scrapes, tooth punctures and bruises are possible as a result of participation in play activities. I (Owner) also understand that pets can receive minor injuries as a result of being groomed (i.e. razor burn, minor cuts or scratches due to sudden movement). I (Owner) understand and agree that Oscar’s Pet Resort, its staff, agents and assigns shall not be liable for any injuries – or problems which may develop from any injuries – that occur while attending Oscar’s Pet Resort, provided that reasonable care, precautions, and instructions are followed (see General Policies). I (Owner) understand that I must notify Oscar’s Pet Resort within 24 – 48 hours of picking up my pet if my pet sustains an injury or change in health during his/her visit.* I agree 10. I (Owner) understand and agree that Oscar’s Pet Resort, its staff, agents, and assigns shall not be liable for any illness, death and/or escape of my pet(s), provided that reasonable care, precautions, and instructions are followed (see General Policies), as a result of my pet(s) attending or participating at Oscar’s Pet Resort.* I agree 11. I (Owner) give my consent to Oscar’s Pet Resort to act in my behalf and in my dog/cat’s best interest by obtaining emergency veterinary care at my expense if deemed necessary. I agree to indemnify and hold Oscar’s Pet Resort harmless for any and all expenses relating to such emergency care.* I agree 12. I (Owner) agree to be financially responsible for any and all costs for continuing care of an abandoned pet(s).* I agree 13. I (Owner) agree to pay Oscar’s Pet Resort’s attorney and court fees in the event an action is filed against Oscar’s Pet Resort in relation to my pet’s stay at Oscar’s and Oscar’s is found not liable.* I agree 14. I (Owner) give Oscar’s Pet Resort the right to use pictures of my pet(s) on Oscar’s Pet Resort’s website, newsletters, and/or other forms of advertising. I (Owner) understand that owner will not be paid any royalty or other compensation and hereby relinquishes any and all rights for payment if a photo or video of the pet is published in any form.* Yes No 15.
