I hereby apply for enrollment for the Sports & Recreation Program at Oscar's Pet Resort. As a condition of enrollment, I understand and certify that: I (Owner) understand that Oscar's payment policy is that payment is due at the beginning of each training session. No refund or credit will be granted after the start of the session. I (Owner) certify that my dog is current on all required vaccinations. I (Owner) understand that the dog Sports & Recreational activities that I will be participating in at Oscar's Pet Resort are defined as any extra-curricular physical dog activity, included but not limited to: walking, running, jumping, twisting, and swimming in varied environments, on varied surfaces, and with the use of a variety of equipment. I (Owner) agree to follow all rules and instruction set forth by an Oscar's Pet Resort Trainer and/or sub-contractor Trainer, including the use of any and all sport equipment. I (Owner) certify that my dog(s) is/are not dangerous to any person or other dogs. I (Owner) understand and agree that my dog's participation in any sports and/or recreational activity could result in injury to Owner and/or dog despite the best instruction, training, and supervision. I (Owner) agree that Oscar's Pet Resort will not be held financially responsible for any injuries to my self (Owner) and/or my dog(s) during any dog sport and recreational activity instruction/participation, whether my dog(s) is/are being handled by myself (Owner) and/or Oscar's Pet Resort employee, Trainer, or any other person. I (Owner) assume the sole responsibility for and agree to indemnify and save the aforementioned parties harmless from any and all loss and expenses (including legal fees) by reason of the liability imposed by law upon any of the aforementioned parties for damage because of bodily injuries, including death, at any time arising out of or in consequence of my participation in Oscar's Pet Resort Sport and Recreational activity program, howsoever such injuries, death or damage to property may be caused, and whether or not the same may have been caused or may have been alleged to have been caused by negligence of the aforementioned parties or any of their employees, Trainers, agents, or any other person. It is recommended the Owner contact the dog(s) veterinarian to seek approval before participation in any sport and/or recreational activity to ensure the dog is medically stable. I (Owner) agree to provide a courtesy cancellation notice of 24 hours, otherwise Owner may be subject to pay the current Training fee. By submitting this form, I agree that I have read and agree to the above.
